Authorization to create and use a synthetic voice replica

This consent is granted by the individual identified below ("Consenting Party"), whose voice will be cloned for the purposes described in Section 3. The Consenting Party may or may not be the same person as the Client of record on the related Master Service Agreement. Both must be identified.
Voice cloning is performed using A.I. In Team's chosen voice AI platform (currently Retell AI, which uses voice synthesis technology from providers including ElevenLabs and PlayHT). The Consenting Party will provide an audio recording (typically 60-180 seconds of clean spoken English) which the platform processes to generate a synthetic voice model capable of producing speech in the Consenting Party's voice across multiple languages.
The resulting voice clone is a distinct digital asset and is hereinafter referred to as the "Voice Clone."
The Consenting Party authorizes A.I. In Team to use the Voice Clone solely for the following purposes:
The Voice Clone will NOT be used for:
Any AI voice agent using the Voice Clone will identify itself as an AI assistant within the first sentence of every call, in compliance with FCC guidance and state laws. The Voice Clone will not be used to impersonate the Consenting Party in any way that would deceive callers about whether they are speaking with a human or with AI.
The Consenting Party may revoke this consent at any time, for any reason, by sending written notice to steven@aiinteam.com. Upon revocation:
Revocation does not affect any agent calls or recordings already produced before the revocation took effect.
The Consenting Party acknowledges that they hold the rights of publicity to their own voice. By signing this consent, the Consenting Party grants A.I. In Team a non-exclusive, revocable, limited license to use their voice solely as described in Section 3, for the duration of the related Master Service Agreement or until revoked, whichever occurs first.
A.I. In Team acknowledges that the Voice Clone remains, in essence, the personal voice of the Consenting Party, and that no broader ownership claim is conveyed by this consent.
Upon termination of the related Master Service Agreement (whether by Client cancellation, Provider termination, or expiration), A.I. In Team will permanently destroy the Voice Clone and source audio within 30 days, unless the Consenting Party has provided new written instructions to retain it for continued service under a new Master Service Agreement.
The Consenting Party agrees to hold A.I. In Team harmless for any consequences arising from authorized uses of the Voice Clone in accordance with this consent. A.I. In Team agrees to hold the Consenting Party harmless for any consequences arising from A.I. In Team's unauthorized use of the Voice Clone outside the scope of this consent.
This consent is governed by the laws of the State of Washington. Any disputes arising under this consent shall be resolved in accordance with the dispute resolution provisions of the related Master Service Agreement, or, if no such agreement exists, in the courts of Kitsap County, Washington.
Both the Consenting Party and the Client must sign and date this form before A.I. In Team will perform any voice cloning. If the Consenting Party and the Client are the same person, both signature blocks should still be completed.
